High School Exchange Student Application

Dear Potential High School Exchange Student,

Thank you for your interest in the Cloud Forest School!  By this time, you have had a chance to review our information packet for visiting students and are ready to apply for admission to either our one semester or a full school year program. Please keep in mind that this opportunity is only available for students who are over 15 years old and who are applying for 10th grade (fall, spring, or full year) or 11th grade (fall semester only).

Please review the application below carefully and only begin to fill it out once you have everything ready (you cannot save an incomplete application). Once your application is complete and sent to us in its entirety, we will be able to consider you as a candidate for our unique exchange program. We will schedule a phone or Skype interview with you shortly thereafter. The student and at least one parent should be present for this interview.

If you are offered placement we will send a formal letter of acceptance, student registration form, and instructions for making a $250 tuition deposit to hold your place in the class.  Please do not hesitate to contact us directly via phone or email with any questions you (or your parents!) may have about this student exchange program. We are here to help make this a pleasant and smooth experience for all involved.

Part 1: File Uploads

Please note that Costa Rica immigration law requires passports to be valid for 6 months from the date of arrival in Costa Rica. If you passport will expire within a year of your arrival to Costa Rica, we recommend you apply for a passport renewal as soon as possible.

Part 5: Permission for Emergency Treatment

On rare occasions, an emergency requiring hospitalization and/or surgery develops. Since minors may not, as a rule be administered an anesthetic or be operated upon without the written consent of the parent of guardian, we request that parents or guardians sign the following statement. Every effort will be made to contact the parents or guardian before any major treatment. This medical release is to prevent a dangerous delay in case an emergency does occur and we are unable to contact parents.
In the event of injury or illness, or if missing vaccinations and health examinations are needed for our son/daughter we hereby authorize the Cloud Forest School/Centro de Educacion Creativa and their officers to secure whatever is deemed necessary, including the administration of an anesthetic and surgery.
